WORKPLACE ACCIDENT INVESTIGATIONS
It is a known fact that accidents can and will happen in the workplace. These incidents can range from minor, near misses, death and everything in between. Unfortunately, if an incident does occur it can have a dramatically negative impact on your business both financially and culturally. We all want safety as a priority in our workplace. We always want our staff and customers to be in a safe environment. However, we know that Incidents can occur, and if they do occur, we want to ensure there is never a repeat of that incident.
Phoenix Fire Training Solutions can come to your premises and conduct a thorough independent and objective investigation of the incident. On completion of the incident investigation a report will be made available to you outlining the systematic causes of the incident and recommendations to lessen the likelihood of the same or similar incident occurring in the future.
The fundamental approach to the investigation is to establish all the systems involved in the incident and to ascertain what influence they may have had regarding the incident. Essentially, I investigate all systems of the business to identify if there are any failures within the system that contributed to the incident.
I use a very well accredited and highly used system of investigation that is utilised in all sectors of business including the mining industry.
This system is called ICAM, which refers to Incident, Cause, Analysis Method.
Whilst there are some businesses that have an active Work Health & Safety programme there are many that do not. Having Phoenix Fire Training Solutions conduct your investigation will ensure a thorough, unbiased and objective approach to conducting the investigation. The culture of the investigation will focus on identifying possible system failure indicators. Our aim is not to identify an individual scapegoat. The vast majority of Workplace accidents occur as a result of the failure in systems. It is a greater benefit for your business if we identify these system failures and rectify them, thus going a long way to ensuring this particular accident does not repeat.
